This has no make on it but looks like something called a Back Friend Support Chair available to buy online.
The two parts will have been attached originally but had come apart when they were given to me. However, I have made good use of them positioned on an office chair for extra back support during long periods of desk work. I found that it provided very good support and was also comfortable.
