Apple iPad screen replacement (Royston EH5)

I went to use my iPad and discovered that I have some how cracked the screen.

Picture shows it’s specifications.

Looking for either two companies that can give me a repair quote on headed paper so I can claim on my insurance or someone that can replace the screen.
