The idea with this device is that you plug it into some audio source (eg. MP3 player, phone, etc), set it to transit on a frequency of your choice then tune your car radio to that frequency to hear the original audio through your car's speaker system.
It comes with its user guide and it's powered by 2xAAA batteries. It used to be able to be powered with a special cable but I think that's long gone.
