Hampton Bay HB-7738-03. (May have been manufactured by Heath Zenith.) Includes mounting hardware and batteries. Works, but for us, there was too much distance between the chime unit & the bell button in our house for it to work reliably. Porch pickup in Downingtown (West Bradford Twp)
