unlocked VOIP ATA adapter (Parkside near Dunkin' Donuts)

I'm moving my home phone number to a provider that does not issue equipment and will need to configure a voip adapter (analog telephone adapter) that accepts input from an ethernet CAT5 cable and puts out a signal to a standard home telephone wiring RJ11. I'd be very happy to take one off of your hands and put it to use.
